RPCNS has moved all of the school enrollment forms, directory and schedules to the Family Portal. You will have access to the Family Portal after acceptance to the school. Access the Family Portal here:
Enrollment Medical Forms -
RPCNS requires additional enrollment forms upon admission. Check the Jovial Family Portal or Admissions Coordinator for more info.
Maryland Health Inventory Form (OCC 1215) - required for every student
(must be filled out by Medical professional)
Immunization Record Form (MDH 896) - required for every student
(must be filled out by Medical professional)
Medical Evaluation for Childcare (OCC 1204) - required for every co-oping adult
(must be filled out by Medical professional)
